Giant juvenile fibroadenoma in a 12-year-old girl treated with breast conserving surgery: Largest reported case in the UK: Association of Surgeons in Training 2018 Conference

Abstract

This case report describes the largest juvenile giant fibroadenoma (GJF) reported in the UK to the authors’ knowledge. Weighing 1600g and measuring 190 x 160 x 65mm. A 12-year-old girl presented with a rapidly enlarging left breast over 2 months in the absence of nipple discharge, pain or skin changes, she did not have a well-defined palpable mass. Ultrasound scan revealed a large lesion (20cm in its largest diameter) with appearance consistent with a GJF.

The lesion was too large to be excised via a peri-areolar incision so an inframammary approach was used. Intra-operative ultrasound was used to locate the margins of the lesion and identify a safe plane to dissect out the GJF reducing risk of damage to the nipple-areolar-complex and skin envelope. The lesion was excised with breast conserving surgery and histologically confirmed as a GJF.

Our patient made an excellent post-operative recovery with a satisfactory cosmetic outcome, although delayed reconstruction does remain a possibility dependent on development of the breast. There is support in the literature to adopt complete surgical excision for definitive treatment and pathological diagnosis owing to the fact an extremely small minority of these turn out to be malignant phylloides tumours on histology.
