Robotic Reconstruction Techniques Following Cystectomy

Abstract

Radical cystectomy with pelvic lymph node dissection remains the gold standard treatment for patients with muscle-invasive bladder cancer who are fit enough to undergo major surgery. However, the morbidity associated with such a procedure is significant at 20-50% and minimally invasive techniques are increasing in popularity in an attempt to reduce these problems. Robot-assisted radical cystectomy (RARC) was first performed in 2003 and although the excisional part of the procedure is now fairly standardized, the same cannot be said of the reconstruction phase. At first, the majority of surgeons performed ileal conduit reconstruction, but an increasing number are now performing orthotopic neobladder formation. It is now possible to undertake the latter technique entirely intracorporeally. This chapter discusses the specific surgical techniques involved, providing a more informed approach to this difficult surgical dilemma.
