The registration of I-123-mIBG SPECT, I-131-mIBG SPECT and CT images for quantitative planning and assessment of I-131-mIBG therapy in a patient with inoperable paraganglioma

Abstract

Image registration involves orienting and scaling multiple images acquired at different times or in different modalities so that they are accurately aligned. Composite registered images may usefully display complementary information; they may for example be used to give anatomical context to functional information from SPECT by incorporating it within physical structures seen in CT, or used to highlight the difference between pre- and post-treatment by using images from the same modality. We have used image registration techniques in conjunction with the Centre of Medical Image Research (CoMIR) at Leeds to quantify the tumour mass and tumour uptake and retention of I-131 meta iodobenzylguanidine (mIBG) in a 34-year-old man undergoing I-131-mIBG therapy for inoperable intraabdominal metastatic paraganglioma. Pre-treatment I-123-mIBG SPECT and CT images were registered in order to assist in treatment planning. I-131-mIBG SPECT images acquired during treatment were used for verification and post-treatment I-123-mIBG SPECT and CT images were registered with the original images in order to quantify tumour response. The relative success of the techniques and the implications for future treatments are discussed.
